Workshop helps facilitate advanced care planning

An advanced care planning workshop to help families deal with future decision-making, especially emotional and financial issues, will take place from 8:30 a.m. to 4:30 p.m. Saturday, Oct. 20, at the Yolo Adult Day Health Center, 20 N. Cottonwood St. in Woodland.

Each participant will receive a notebook in which to gather information to help develop a care plan for someone they care for or for themselves. The plan will include information about one’s support system, important papers and information regarding health, finances, legal concerns, housing options, reorganizing, end-of-life issues and one’s life story, as well as resource information.

The day will include information, discussion, and an opportunity to begin the care planning process. It will be facilitated by Peggy Phelps, caregiver resource specialist.
