A series of workshops focused on advanced care planning — making decisions about future health care for oneself or a loved one — will begin this week. Each workshop runs from 6 to 8 p.m. at Woodland Christian Church, 509 College St. in Woodland, and features presenter Peggy Phelps, a caregiver resource consultant with the Yolo Adult Day Health Center and Woodland Healthcare.
There is no cost, although, donations for materials are appreciated. Sessions are:
* “Advanced Care Planning” on Wednesday, which will include information about a person’s family and support system; planning for emergencies; aging at home; important papers and information gathering;
* “Health Information” on July 17, assessing one’s medical conditions, medications, functional abilities, care providers and insurance needs;
* “Legal and Financial Planning” on July 24, focusing on legal documents, financial concerns, estate planning, housing needs and reorganizing personal possessions; and
* “Final Arrangements”on July 31, looking at end-of-life wishes, care options, and affordable and appropriate funerals and cemetery options. A survivor’s first-year responsibilities also will be discussed.
